No animation with heavy attack with sword [Solved]

When doing a heavy attack with a sword (attack set id 1), I hear the woosh sound, stamina is reduced, but the character does no animation.  Doesn't seem to damage any enemies either.  The example scene/demo provided is also doing this as well.
